Meet the Team

Our team consists of faculty, administrators, clinic and hospital staff who work closely with residents through frequent collaboration over the course of their three years in training in our program. We understand that wellness is an expectation powered by supportive camaraderie and the knowledge that we can support people in being as well as possible while adapting to residency. Residents learn how to partner with their team to provide high-quality care. We believe that everyone should work to the top of their license and we implement processes that facilitate this.

Onsite Pharmacy Team

We have a PharmD on Faculty and a second-year ambulatory care pharmacy resident as part of our clinical team. They are integral to resident education and patient care to help co-manage patients with complex medical issues and polypharmacy. Their expertise includes but is not limited to diabetes, heart failure, COPD/Asthma, and psychiatric conditions. They are available on site for consults, topic discussions, and patient care in the form of joint visits. Residents can also elect for additional shadowing opportunities.

Onsite Behavioral Health Specialist

Our faculty Behaviorist is a Licensed Clinical Social Worker. The behaviorist works as part of the interdisciplinary team to help co-manage complex patients. The behaviorist is available on-site to see patients for individual counseling, in joint visits and is available for curbside consults. The behaviorist also assists in collaborating with local agencies to help patients navigate various community resources.

Onsite Community Health Worker

We have an onsite Community Health Worker who comes to the clinic once per week to help support our patients and provide resources for housing, food insecurity, transportation, and numerous other services both inside and outside the community. They are available five days per week for referrals and consults. We are grateful that our patients have this essential member of the team to help them in navigating the health care system and reduce social and economic barriers to getting the care they need.
