Sacred Heart Medical Center and Children’s Hospital (SHMC) is a mission-based, not-for-profit health care system in Spokane, WA. At 644 beds, it is the largest medical center in the Inland Northwest and the largest between Seattle and Minneapolis. It is a referral center for much of the region and has a catchment region extending 300-400 miles. SHMC serves an ethnically and socioeconomically diverse population that is representative of the community and region. All major psychiatric and medical diagnoses are represented given the large catchment. Regarding medical care, only burns, extensive trauma, and certain organ transplants are referred to Seattle, otherwise all major medical conditions are served at SHMC.

Spokane Teaching Health Clinic is a state of the art, interprofessional clinic, fulfilling two main aims; a focus on excellent in clinical care, and true integration in primary care specialties embedded with pharmacy, legal aid, physical and occupational therapy, social work, and on-site laboratory and imaging services. Psychiatry residents and fellows work along residents in Family Medicine, Internal Medicine, and Rural Family Medicine, as well as medical, pharmacy, and social work students.
Nearly 60% patients have either medicaid or medicare insurance: 30% patients have medicaid insurance, 27% have medicare insurance. They are often patients who are not eligible to receive services in other settings, such as the community mental health clinic (because they have moved to medicare insurance) or in the private practice setting. The clinic provides an Integrated Behavioral Health to Family Medicine and Internal Medicine Spokane Residencies whose outpatient clinics are also located at this site. Psychiatric consultation is performed in a collaborative care model. It is also the site for a 6 month required PGY3 addiction psychiatry rotation, in a newly established opioid treatment clinic. Residents practice in this setting for 3 years of their four year training, as part of a team that practices patient centered, outcome based integrated psychiatric care in the primary care setting. Residents learn collaborative care psychiatry with adults, and children and adolescents, and learn telepsychiatry consultation to a rural county with the main town, Colville, 70 miles north of Spokane. Residents provide indirect telephone consultation, direct patient consultation and ongoing treatment of chronically mentally ill patients, being expected to provide 2.5 hours of direct patient care per ½ day. Main diagnoses are depression, anxiety, personality disorders, psychotic disorders, bipolar disorders, adhd and substance use disorders. Site support is provided by five psychiatrists: Amy Burns MD, who serves as site director for this location; Lucas Salg MD, Tanya Keeble MD, Kelly Wurzel MD, John Wurzel MD, with psychology supervision from Robert Maurer, PhD.

Eastern State Hospital - ESH is a 317 bed inpatient psychiatric hospital and is accredited by the Joint Commission and certified by the federal Centers for Medicare and Medicaid Services - CMS. ESH is located in the city of Medical Lake, Washington, which is approximately 20 miles west of Spokane, Washington. ESH is one of two state-owned psychiatric hospitals for adults in the state of Washington.
Adult Psychiatric Unit (APU) - APU is a 91 bed unit that provides inpatient hospitalization for adults 18 to 50 years old who are severely mentally ill and are committed for evaluation and treatment by a civil court proceeding. Geropsychiatric Unit (GPU) - GPU is a 101 bed unit that provides inpatient psychiatric evaluation and treatment for individuals 50 years of age and older, or persons under 50 years of age with medical concerns. Forensic Services Unit (FSU) - FSU is a 95 bed inpatient unit for patients who enter the forensic (legal) unit in the hospital through the criminal justice system. Evaluation and treatment services are provided for adults prior to their trial, after they are convicted, or after they are acquitted by reason of insanity.

Frontier Behavioral Health (FBH) is a Community Mental Health Center. It is a non-profit organization serving the Spokane region for more than 100 years. FBH has approximately 500 staff at 13 service sites throughout Spokane County and serves over 12,000 individuals and families each year. It is the Lead Service Provider in the Spokane County Regional Support Network (SCRSN) and collaborates with over 100 other organizations, mental health, and substance abuse treatment providers throughout the community. FBH provides comprehensive, multidisciplinary community mental health care for children, adults, and elders with complex mental health problems for a broad range of patients in all diagnostic categories. The majority is of lower socioeconomic status and minorities are over-represented relative to the community. 95% of the patients served by this organization have Medicaid or Medicare insurance. It provides case management, individual, group and family therapy and psychiatric and psychological services.

Gonzaga University population includes undergraduate students and graduate students in programs such as Gonzaga Law School and the University of Washington Medical School. Gonzaga University provides psychotherapy continuity clinic rotations through Health and Counseling Services during PGY3 and PGY4. Starting 2020, it is a site for PGY4 primary care psychiatry elective, an opportunity to provide psychiatric treatment in the medical clinic. Residents are supervised by a full-time board certified psychologist, and by core program psychiatry faculty. Diagnoses are primarily depression, anxiety and personality disorders.
