Application Process
Requirements for Acceptance
At minimum, the candidate must meet the following criteria to be considered
for selection:
- Graduate or candidate for graduation from an Accreditation Council for
Pharmacy Education (ACPE) accredited degree program (or one in process
of pursuing accreditation) or have a foreign pharmacy Graduate Equivalency
Committee (FPGEC) certificate from the National Association of Board of
Pharmacy (NABP)
- Licensed or eligible for licensure in the state of California within 90
days of the beginning of the residency program
- Submit completed application through PhORCAS by due date and register for
the ASHP MATCH program
How to Apply
Providence Cedars-Sinai Tarzana Medical Center participates in the ASHP
PhORCAS electronic application service. Applicants must submit all documents
below through
PhORCAS before January 2:
- Letter of intent

A brief essay describing the following:
- How will PCSTMC’s residency program help further your professional goals?
- As a prospective resident, how can you contribute to PCSTMC’s program?
- Official transcripts from College of Pharmacy
- Curriculum Vitae
- Three letters of recommendation (at least one from a faculty member and
one from employer)
As an ASHP accredited program, Providence Cedars-Sinai Tarzana Medical
Center requires the resident candidate to participate in the ASHP MATCH
program. Personal interviews will be scheduled from the end of January
through end of February. Selected applicants will be notified of their
interview times via email.
