*Please note: We are one program with two sites. Candidates may apply to both PPMC and PSVMC sites for the PGY1 Pharmacy program for a single PhORCAS application fee. Candidates are encouraged to rank both sites during the ranking process.

For your application to be complete, all of the following materials must be received through Pharmacy Online Residency Centralized Application Service (PhORCAS) before the due date:

  • Letter of intent (maximum 500 words) explaining why you’re seeking a residency at Providence Health & Services
  • Current curriculum vitae
  • Official transcripts from a college of pharmacy
  • Three completed reference recommendation forms within PhORCAS

You will be notified by the end of January if you are selected for an interview.

PGY1 Managed Care Residency Program

PGY2 Ambulatory Care Pharmacy Residency

Successful completion of an ASHP accredited PGY1 residency is required.

You may apply directly with ASHP for an exemption if you meet the following criteria:

  • Have practiced at least three years as a pharmacist
  • Are licensed to practice as a pharmacist in the United States
  • Have postgraduate experience that may be equivalent to PGY1 training

ASHP will review the application and determine your eligibility for the match. Please contact the residency director if you meet the qualifications above and want to pursue this option.
