When you have the forms with the required signatures completed, please e-mail them to the contact from the list based on the clinical setting where you will be rotating. That contact will complete the onboarding process.
You will receive an e-mail from them when you have been cleared to start your rotation. You cannot start your rotation until you get confirmation.
