Graduation Requirements
To graduate from the residency program, the resident must have completed
the following:

All required forms must be completed and submitted to the residency coordinator
throughout the residency and upon completion of the residency program.
This includes but is not limited to:
- Mentor prep forms
- All residency related evaluation forms
- Medical conditions tracking forms and outcomes
- Mentoring hours tracking forms
- Passing of all written and live patient examinations
- Demonstrates progress per the mentor rated PTRCEI achieving level 4 in
80% of the core competencies.
- Demonstrates satisfactory or superior performance in 80% of the assigned
skills check list.
- Completion of residency education activities inclusive of duties assigned
with affiliated parties (i.e. Pacific University and Orthopedic and Fracture
specialists).
- Participated in a community education event that is agreed upon by the
resident and their coordinator.
