Graduation Requirements

To graduate from the residency program, the resident must have completed the following:

• All required forms must be completed and submitted to the residency coordinator throughout the residency and upon completion of the residency program. This includes but is not limited to:
  • Mentor prep forms
  • All residency related evaluation forms
  • Medical conditions tracking forms and outcomes
  • Mentoring hours tracking forms
• Passing of all written and live patient examinations
• Demonstrates progress per the mentor rated PTRCEI achieving level 4 in 80% of the core competencies.
• Demonstrates satisfactory or superior performance in 80% of the assigned skills check list.
• Completion of residency education activities inclusive of duties assigned with affiliated parties (i.e. Pacific University and Orthopedic and Fracture specialists).
• Participated in a community education event that is agreed upon by the resident and their coordinator.
