Admission Requirements
-
A high school diploma/equivalent or highest level of completed schooling
from an accredited institution is required for admission.
- Candidates must be at least 18 years of age and eligible to work and live
in the U.S.
- Candidates with current AHA BLS certification are more competitive in the
application process.
- In-person personal interview to assess professional goals, judgment, cognitive
ability, interpersonal skills and communication skills required for the
profession.
Please Note: A successful background check and drug screen are required for acceptance.
Proof of vaccination is required. All qualified applicants are considered.
Applicant acceptance is determined by the Providence Phlebotomy Training
Program Admissions Committee.

How to Apply
Applications and Interest Forms are accepted year-round for multiple training
cohorts. If currently in an interview cycle for an upcoming cohort, the

those who complete the Interest Form will be notified when the next interview
